Tax Forms
If you plan on purchasing tax-exempt products:
- Fill out an MWI/AML Tax Classification Form (32KB).
- Fill out the appropriate Sales Tax Exemption Form(s) from the table below for the state in which your practice is located. MWI must charge sales tax unless you provide us with a copy of your Tax Exemption Certificate.
- Send the forms to MWI using one of the following options:
